Submit a Claim

To submit a claim for your mobile device, fill out the form below:

  • Claims should only be submitted on phones that are active as shown on your mobile device wireless bill for the month prior to the date of damage or theft.
  • Coverage for damage begins the first day of the month following the date you became eligible. There is no waiting period for theft. Phones that are lost or “mysteriously disappear” are not covered.
  • All required documentation for your claim must be submitted within 90 days of the incident. If additional information is requested, it must be returned within 6 months of the incident date.
